  I, the undersigned, consent for myself or my child, named above, to participate in the St. George Fair Dragon Shootout Saturday, October 7^th or Sunday, October 8^th. Myself or my child has no known physical or other condition that would limit or restrict participation. I understand that while participating in this sporting event there is a possibility that myself or my child may sustain physical illness or injury (minimal, serious or catastrophic). I further understand that as a guardian, my child and/or I assume the risk of physical illness or injury, and I release St. George Catholic Church and School as well as its representatives, from any claim for such illness or injury that myself or my child may sustain while participating in the St. George Fair Dragon Shootout In order that myself or my child may receive the necessary medical treatment for injury or illness sustained while participating in the event, I authorize the responsible parties to obtain medical treatment for myself or my child for such injury or illness, and I hold St. George Catholic Church and School as well as its representatives, harmless in exercise of this authority. I understand that I will be responsible for any medical bills that may be incurred on my behalf of myself or my child for physical illness and injury that myself or my child may sustain while participating in the St. George Fair Dragon Shootout, I certify that my and/or child’s activities in this event are covered under our personal accident and health insurance.
